中药灯心草作栓塞剂对肾肿瘤术前栓塞的应用研究

目的探讨中药灯心草作栓塞剂对肾肿瘤术前栓塞的应用。方法将直径0．2cm的长条状灯心草剪成长1cm的段包好，高压消毒备用。采用Seldinger方法，经右侧股动脉穿刺插管至病变侧肾动脉数字减影，了解肿瘤供血情况。灯心草在碘油或76％的泛影葡胺中浸泡，然后放入注射器前端推入所选择的动脉。所用数量根据栓塞情况而定，直至满意为止。结果4例肾肿瘤栓塞后重复造影，均显示栓塞效果满意，其中3例低热并肾区胀痛，对症处理后缓解。栓塞后3天手术，见肾脏缩小，呈紫色，肾动脉栓塞变硬，术中出血不多。结论通过本文4例试用，认为中药灯心草作栓塞剂有取材方便、价格低廉；易于加工制作；有柔软膨胀的海绵状特性，无毒等特点，适合做大的动脉栓塞。肾肿瘤术前栓塞的目的在于使肿瘤缩小，为手术切除创造条件，减少术中出血。

The study of preoperative embolization of renal tumor with rush -herb

Objective To explore the usefulness of a traditional Chinese medicine - rush in embolizing renal tumors preoperatively. Methods The herb,0.2cm in diameter,was cut into segments of 1 cm in length, and then sterilized. Renal subtract angiography of the kidney with tumor was done via right femoral artery using Seldinger Method in order to show vascular structure of the tumor. The herb was soaked in the iodine oil or 76% Urografin solution,then put into syringe and injected into the selected arteries for embolization. Results Renal angiography was repeated in 4 cases following embolization,indicating that embolization was sufficient as expected. Low fever and renal area pain occurred in 3 of these 4 cases, and relieved after systematic treatment. During surgery done three days after embolization,it was found that the embolized kidney became decreased in size, blue in color,and the embolized arteries were hardened,and bleeding was remarkably reduced. Conclusion The herb as an embolizing agent has such advantages as convenient to obtain,cheap and easy to process,soft and inflated like sponge and no toxicity,suitable for embolization in large arteries. Preoperative embolization can make the tumor decreased and reduce the bleeding in operation.
