超声介入在治疗肝海绵状血管瘤中的应用

目的探索超声引导下对肝脏海绵状血管瘤经皮肝穿刺介入疗效的临床价值。方法48例肝血管瘤患者,在超声引导下经皮穿刺瘤内注入平阳霉素与1%利多卡因混合液,共治疗62个瘤结节,肿瘤直径3.0-8.5cm,平均（5.8±0.7）cm。每个结节注射1-2次,术后3、6个月行超声复查。结果彩色多普勒血流超声检查在病变内或周边未探及血流信号。48例患者62个瘤体中29个瘤体基本或完全消失,瘤体缩小20%以上者33个,随访无增大现象。治愈率达29/62（46.8%）,显效率33/62（53.2%）,总有效率为100%。结论介入超声治疗肝血管瘤并发症少,是一种安全、简便、有效的诊疗方法,值得推广应用。

Application of ultrasound - guided percutaneous pingyangmycine injection therapy for hepatic cavernous hemangioma

Objective： To explore the value of percutaneous intra - temporal injection of Pingyangmycine for hepatic cavernous hemangioma in clinical practice. Methods ： Forty - eight patients of hepatic hemangiom cavernous with 62 tumor nodules were treated by intra - tumor injection of mixed liquid of 1% Lidocaine and Pingyangmycine under ultrasound guidance. The diameter of lesions was ranged from 3 to 8.5 cm （ mean, 5.8±0.7 cm）. Each lesion was treated with one or two sessions. Three and six months after the operation, the patients were rechecked by ultrasound to evaluate the curative effect. Results ： In the 48 patients with 62 tumor nodules were followed up, 29 lesion disappeared basically or completely in half a year with the cure rate 46.7% , and 33 lesion were smaller than 20% in diameter with the effective rate 53.3%. None of them enlarge, the total effective was 100%. The blood flow inside or around the lesions could not be detected by CDFI. Conclusion ： Ultrasound - guided pereutaneous Pingyangmycine injection therapy for hepatic cavernous is able to destroy the tumor nodules, with less complication, and is easy to operate, which is valuable for wide application.
