三维超声对肝脏良恶性肿瘤的鉴别诊断价值

目的探讨三维超声在肝脏良恶性肿瘤鉴别诊断中的应用价值。方法196例肝占位患者，恶性肿瘤120例、良性肿瘤76例，对肿瘤进行三维超声成像，分析两组肿瘤中心、外周及穿支血流分布情况。结果恶性肿瘤外周、中心及穿支血流的比例分别为87．5％、96．7％和72．5％，而良性肿瘤为84．2％、25．0％和6．6％。两组间的外周血流分布无统计学差异（P〉0．05），恶性肿瘤中心及穿支血流的分布则明显高于良性肿瘤（P〈0．05）。结论三维超声可为良恶性肿瘤提供重要的血流信息，有助于两者的鉴别诊断。

The Value of Three-dimensional Ultrasound for Differential Diagnosis of Liver Neoplasm

Objective To discuss the value of dynamic three-dimensional ultrasound for differential diagnosis of liver neoplasm. Methods 196 cases with liver neoplasm underwent three-dimensional ultrasound, and the blood flow signals inside the tumor, parameters and branches were detected. Results The frequency of distribution of the blood flow signals inside the tumor, parameters and branches in malignant group were 87.5%, 96.7% and 72.5%, while 84.2%, 25.0%, 6.6% in the benign group. There was no statistical significance between the two groups in the distribution frequency of parameters blood flow, but the difference was statistically significant in that of inside and branches. Conclusion Three-dimensional ultrasound can provide important and different signals of blood flow in malignant and benign tumors, and this is valuable to make the differential diagnosis between them.
