尿胰蛋白酶原—2快速检测筛选急性胰腺炎的临床价值

目的：评价试纸法尿胰蛋白酶原－2快速检测诊断急性胰腺炎的临床价值。方法：对109例急性腹痛病例进行尿胰蛋白酶原－2及血、尿淀粉酶检测，并比较其敏感性和特异性。结果：54例急性胰腺炎患者中，52例尿胰蛋白酶原－2阳性，敏感性为96％；55例其他原因所的急性腹痛患者中，4例尿胰蛋白酶原－2阳性，特异性为93％。而血清淀粉酶检测（断点值为300U／L）的敏感性为87％（47／54），特异性为85％（47／55）；尿淀粉酶检测（断点值为2000U/L)的敏感性和特异性分别为81％（44／54）和78％（43／55）。尿胰蛋白酶原－2快速检测的敏感性和特异性均显著高于血、尿淀粉酶检测（P＜0．05）。结论：试纸法尿胰蛋白酶原－2快速检测诊断急性胰腺炎具有较高的精确性，是筛选急性胰腺炎简便而快速的方法。

Clinical Value of Rapid Measurement of Urinary Trypsinogen-2 in Screening Acute Pancrea titis

Background/Aims: To investigate the clinical value of rapid measurement of urinary trypsino- gen-2 in screening acute pancreatitis. Methods: Comparison between rapid measurement of urinary trypsinogen-2 and serum and urinary amylase test was made in 109 acute abdominal pain patients. Re- sults: Urinary trypsinogen-2 was positive in 52/54 patients with acute pancreatitis, and in 55 patients with acute abdominal pain due to other causes, urinary trypsinogen-2 was positive in 4 cases, the sen- sitivity and specificity were 96% and 93%, respectively. The sensitivity and specificity of serum amy- lase test (cut-off level 300 U/L) were 87% (47/54) and 85% (47/55), respectively, and urinary amylase test (cut-off level 2 000 U/L) 81% (44/54) and 78% (43/55), respectively. The sensitivity and specificity of rapid measurement of urinary trypsinogen-2 were significantly higher than those of serum and uri- nary amylase test. Conclusions: Rapid measurement of urinary trypsinogen-2 is easy to perform with good precision in screening acute pancreatitis.
