雌激素孕激素与宫颈癌关系的病例对照研究

目的探讨内源性雌激素、孕激素在宫颈癌发生中的作用。方法采用以医院为基础的配比病例对照研究方法。选择山西省肿瘤医院宫颈鳞癌新发病例74例作为病例组,以1∶1配比的方式选择同期同院确诊的宫颈良性肿瘤患者作为对照组,两组配比条件为年龄、民族、入院时间一致,病例与对照均需排除孕妇、其他癌症患者、1年内口服避孕药和其他甾体类激素使用者。应用酶联免疫吸附试验(ELISA)定量测定血清雌二醇(E2)和孕酮(P)水平。结果E2与宫颈癌的关联性具有统计学意义(P<0.01);但未发现P水平与宫颈癌之间的统计学关联;E2、P水平与宫颈癌之间存在剂量反应关系。结论内源性雌激素、孕激素水平升高可增加宫颈癌发生的危险性,而E2升高的危险性更强,且不受绝经因素的影响。

Case-control study on relationship between estrogen,progesterone and cervical cancer

Objective To investigate whether the physiologic levels of endogenous female hormones,such as estrogen,progesterone predispose,increase the development of cervical cancer.Methods Hospital-based case-control study in the Tumor Hospital of Shanxi Province was performed.There were 74 women with invasive cancer and controls with hysteromyoma,which were matched by 1∶1 ratio.All controls were matched to tumor patients by age,nationality and length of stay.Estrogen and progesterone were detected by ELISA.Results The levels of estrogen were significantly higher in tumor patients than in controls(P<0.01).It showed a dose-dependent direct association.Conclusion There is a clear trend of increasing risk of cervical cancer with increasing levels of estradiol and progesterone,and serum levels of estradiol may increase the risk of cervical cancer.
