猴食道和舌上皮用于IIF检测寻常型天疱疮抗体的研究

摘    要:目的:比较猴食道上皮(MO)和猴舌上皮(MT)用于检测寻常型天疱疮抗体的差异及猴食道上皮为底物检测的抗体滴度与疾病严重度的相关性。方法:采用MO和MT为底物的间接免疫荧光法(IIF)分别检测寻常型天疱疮患者血清中抗体。结果:以MO和MT为底物检测寻常型天疤疮患者血清抗体的灵敏度分别为84.1%和93.2%,特异度分别为82.8%和93.1%,两种底物间接免疫荧光检测方法差异无显著性(p〉0.05)。猴食道上皮(MO)为底物间接免疫荧光滴度与口腔黏膜病损严重程度相关(p〈0.01),而与皮肤损害严重程度无相关性(p〉0.05)。结论:以猴食道/猴舌上皮为底物IIF法检测寻常型天疱疮抗体均有较高的灵敏度和特异度,可做为寻常型天疱疮的常规诊断方法之一。由于猴食道上皮阳性荧光形态明显,更适用于临床应用。以猴食道上皮为底物检测的抗体滴度与口腔黏膜病损严重程度相关。

Abstract:Objective:To compare the difference of sensitivity between monkey oesophagus(MO) and monkey tongue(MT)used as substrates in detection of pemphigus vulgaris antibody and the relationship between the antibody titers and the disease activity. Method:Antibody titers were detected by indirect immunofluorescent assay (IIF) on MO and MT substrates respectively.Result:The sensitivity of IIF was 93.2% used as monkey oesphagus as a substrate and 84.1% on monkey tongue as a substrate. The specificity of IIF was 82.8% on monkey oesphagus and 93.1% on monkey tongue. There was no significant difference of the sensitivity and specificity between the two substrates (p>0.05).Antibody titers detected by MO substrate showed positive correlation with oral disease severity(p<0.01) and no correlation with skin disease severity in pemphigus vulgaris(p<0.05).Conclusion:The data presented in this study suggest that MO/MT IIF method were highly sensitive and specific in diagnoses of PV. But MO is more suitable for clinical usage. Antibody titers detected by MO substrate showed positive correlation with oral disease severity
