慢性心力衰竭患者血清瘦素、脂联素表达水平的临床研究

目的探讨慢性心力衰竭（CHF）患者血清瘦素、脂联素表达水平的变化。方法选取桂林医学院附属医院2011年2月~2012年12月收治的78例慢性心力衰竭患者为研究组,选取同期非慢性心力衰竭且心功能均为Ⅰ级的40例患者为对照组,检测两组患者的血清瘦素、脂联素、血糖、低密度脂蛋白胆固醇（LDL-C）、高密度脂蛋白胆固醇（HDL-C）、血清总胆固醇（TC）以及三酰甘油（TG）,并进行统计学比较。结果两组患者的性别、年龄、血压以及心率差异均无统计学意义（t=1.231、1.334、1.811、1.025、1.781,均P〉0.05）。研究组患者的血清瘦素、脂联素与对照组比较,差异有统计学意义（t=2.984、3.256,P〈0.05）。两组患者的餐后血糖、空腹血糖、LDL-C、HDL-C、TC以及TG比较,差异无统计学意义（P〉0.05）。Ⅱ、Ⅲ、Ⅳ级心功能患者的间血清瘦素、脂联素水平差异有统计学意义（P〈0.05）。结论血清瘦素、脂联素与慢性心力衰竭患者的病情发展密切相关,可以用于临床检测慢性心力衰竭患者的病情进展。

Clinical research of blood leptin, adiponectin expression level in chronic heart failure patients

Objective To explore the change of blood leptin,adiponectin expression level in chronic heart failure pa tients.Methods Selected seventy-eight cases of patients with chronic heart failure who were treated in Affiliated Hospital of Guilin Medical University from February 2011 to December 2012 as the research group,40 patients with no chronic heart failure and cardiac function grade Ⅰ at the same period were selected as the control group,the blood leptin,adiponectin,blood sugar,low density lipoprotein cholesterol（LDL-C）,high density lipoprotein cholesterol（HDL-C）,serum total cholesterol（TC） and three acyl glycerin（TG） of two groups of patients were detected,and they were for statistical compared.Results The gender,age,blood pressure and heart rate between the two groups of pa tients were compared,there was no statistically significant difference（t = 1.231,1.334,1.811,1.334,1.811,P ＆gt; 0.05）.The team blood leptin,adiponectin in research group compared with the control group,the difference has statistical significance（t = 2.984,3.256,P ＆lt; 0.05）.The postprandial blood sugar,fasting glucose,LDL-C,HDL-C,TC and TG were compared between the two groups,the difference has no statistical significance（P ＆gt; 0.05）.The blood leptin,adiponectin levels among the Ⅱ,Ⅲ,Ⅳ grade of cardiac function patients,the difference was statistically significant（P ＆lt; 0.05）.Conclusion The blood leptin,adiponectin and development is closely related to the condition of patients with chronic heart failure,they can be used for clinical detection of patients with chronic heart failure progression.
