输尿管梗阻致肾包膜下积液8例报告

目的:探讨输尿管梗阻致肾包膜下积液的原因与处理办法。方法:报告8例此种患者的临床资料。8例患者均经影像学检查确诊;输尿管结石所致者5例,外来肿瘤压迫输尿管、良性疾病手术后输尿管受压和放疗所致输尿管梗阻者各1例。针对梗阻原因分别行输尿管逆行插管、碎石、手术探查及肾包膜下穿剌等治疗。结果:8例患者输尿管梗阻解除后肾包被膜下积液均消失,肾功能恢复正常。结论:肾包膜下积液大多由泌尿系结石致梗阻而引起,也可由输尿管附近病变压迫输尿管致管腔梗阻所致。影像学检查对本病的诊断具有重要作用,解除梗阻后肾功能常恢复良好。

The subcapsular fluid collection of kidney in patients with ureteral obstruction (Report of 8 cases)

Objective:To explore the etiology and treatment of subcapsular fluid collection of kidney in patients with ureteral obstruction.Methods:We analyzed the clinical data of subcapsular fluid collection of kidney in 8 patients with ureteral obstruction. Eight patients with subcapsular fluid collection of kidney were all diagnosed according to radiology. There were ureteral stones in 5 patients. The ureter was obstructed because of this ureter compressed by the mass resulting from the extraureteral carcinoma in one patient. There was one patient whose unilateral ureter was compressed by the mass resulting from postoperation because of benign disease. There was one patient whose unilateral ureter was obstructed after radiological treatment because of cancer. Retrograde double J stents were put into ureters . Ureteroscope pneumatic lithotripsy or open surgery was performed. And the subcapsular fluid collections were pumped out during operation or preoperation.Results:The subcapsular fluid collections disappeared after the ureteral obstruction was resolved in 8 patients. The renal function come to be normal.Conclusions:The cause of the subcapsular fluid collection is mainly ureteral obstruction in patients with ureteral stones. Sometimes, there is also another reasion why the ureter can be compressed and obstructed by the mass located in the extraureter. Radiologic imaging plays an important role in the diagnoses of subcapsular fluid collection and ureteral obstruction. The renal function usually restore to be normal after ureteral obstruction is resolved.
