腹腔镜鞘膜内和辅助阴式子宫切除术的疗效分析

目的：评价腹腔镜鞘膜内子宫切除术（LISH）和腹腔镜辅助阴式子宫切除术（LAVH）2种术式的临床效果。方法：回顾性分析妇产科2007年6月～2008年12月分别行LISH和LAVH的140例患者，在手术时间、出血量、术后恢复等方面情况。结果：平均手术时间、术中平均出血量、术后性欲降低方面LISH组与LAVH组比，差异有统计学意义（P〈0．05）；术后使用抗生素时间、术后住院时间、术后疼痛及术后恢复日常活动时间LISH组与LAVH组比，均差异无统计学意义（P〉0．05）。结论：LISH和LAVH两种术式均为腹腔镜下子宫切除的有效术式，且各有利弊；应根据患者的具体情况选择适宜术式。

Comparison of laparoscopic intrafascial supracervical hysterectomy and laparoscopic-assisted vaginal hysterectomy

Objective:To evaluate laparoscopic intrafascial supracervical hysterectomy(LISH) and laparoscopic-assisted vaginal hysterectomy and laparoscopic-assisted vaginal hysterectomy(LAVH).Methods:Retrospectively reviewed clinical data of 140 patients who underwent LISH and LAVH between June 2007 and December 2008 in our hospital,the operation time,blood loss and postoperative recovery were studied.Results:Compared patients undergoing LAVH and LISH for mean operative time,average amount of bleeding,and libido decline,the differences were significant(P<0.05);compared patients who underwent LAVH and LISH for postoperative use of antibiotics,postoperative hospitalization time,postoperative pain and resumption of daily activities,the differences were not significant(P>0.05).Conclusion:Both LISH and LAVH are effective surgical procedures with advantages and disadvantages,which may be used according to the concrete conditions of patients.
