37例基底动脉尖综合征临床分析

目的 探讨基底动脉尖综合征(TOBS)的临床特点.方法 回顾性分析我院2005年2月-2012年2月收治的37例TOBS临床资料,观察其危险因素、临床表现、影像特点及预后.结果 TOBS老年人多见,其主要危险因素有高血压病(21例)、高脂血症(16例)、吸烟(12例)、脑梗塞史(9例)、心房纤颤(6例),其临床症状、体征呈多样化特点,多以眩晕(17例,占45.9％)、意识障碍(11例,占29.7％)、眼球运动障碍(13例,占35.1％)、皮质盲或偏盲(12例,占32.2％)起病.磁共振显示累及脑干(16例)、丘脑(16例)、小脑(12例)、枕叶(13例)、颞叶(11例)的多发梗死病灶.结论 TOBS临床表现复杂多样,预后差,临床医师应提高警惕,尽早完善MRI检查避免误诊及漏诊.

Clinical analysis in 37 patients with top of the basilar syndrome

Objective To investigate the clinical characteristics of top of the basilar syndrome.Methods The clinical datas of 37 patients with top of the basilar syndrome in our hospital from February 2005 to February 2012 were retrospectively analyzed to observe the risk factors,clinical manifestation,image characteristics and prognosis.Results TOBS mostly occurs in aged people.The main risk factors had hypertension(21 cases),tall lipemia(16 cases),smoking(12 cases),cerebral infarction history(9 cases),atrial fibrillation(6 cases),their clinical symptoms,signs were diversified characteristics.The clinical symptoms,signs were diversified characteristics,with vertigo(17 cases,45.9%),disorder of consciousness(11cases,29.7%),eye movement disorder(13 cases,35.1%),cortical blindness or hemianopia(12 cases,32.2%) onset.Magnetic resonance imaging showed the brainstem involvement(16 cases),thalamus(16 cases),the cerebellum(12 cases),the occipital lobe(13 cases),temporal lobe(11 cases) of the multiple infarction lesions.Conclusion TOBS clinical manifestation is complicated,prognosis is poor,clinical physicians should be vigilant,perfect the MRI as soon as possible to avoid misdiagnosed.
