超声斑点追踪显像评估原发性高血压病患者的左室功能

摘    要:目的 探讨斑点追踪显像(STE)技术评价原发性高血压病患者左室二维应变和扭转变化规律的价值.方法 60例原发性高血压病患者,30例为左室正常构型(LVN),30例为左室重构(LVR),另选取健康者30例为对照组,应用STE技术测量并计算左室整体纵向应变(GLS)、整体径向应变(GRS)、整体圆周应变(GCS)、峰值扭转角度(Ptw)、扭转角度达峰时间(t-Ptw)、等容解旋率(IUR)、收缩期扭转峰速度(PTV)及其达峰时间(tPTV)、舒张期解旋峰速度(PUTV)及其达峰时间(tPUTV).结果 与对照组比较,LVN组及LVR组的GLS、IUR减低,Ptw、PTV、PUTV增加(P〈0.05);tPtw有延长趋势,GCS有增加趋势(P〉0.05);此外,LVR组的GRS减低,tPUTV延长(P〈0.05).与LVN组比较,LVR组的IUR、GLS进一步减低,GRS减低,Ptw、PTV、PUTV进一步增加,tPUTV延长(P〈0.05).结论 STE技术通过评估原发性高血压病患者左室的二维应变和扭转特征,可敏感反映左室功能的变化.

Abstract:Objective To explore the value of speckle tracking echocardiography(STE) in assessing left ventricular two -dimensional strain and twist in patients with essential hypertension (EH) . Methods Thirty EH patients with left ventricular nonnal geometry (LVN), thirty EH patients with left ventricular remodeling(LVR) and thirty normal controls were em'olled in this study. Global longitudinal strain (GLS), global radial strain (GRS), global circumferential strain(GCS), global peak twist angle (Ptw), time to peak twist(t -Ptw), Isovolumic untwisting rate (IUR), peak twisting velocity (PTV), time to peak twisting velocity(tPTV) ,peak untwisting velocity(PUTV) and time to peak untwisting velocity were measured and calculated by STE software. Results Compared with the control group,the value of GLS, IUR were significantly decreased, Ptw, PTV, PUTV were significantly increased in LVN group and LVR group ( P 〈 0.05 ), and GRS was significantly reduced, tPUTV was prolonged in LVR group( P 〈0.05 ). There was only a prolonged trend in tPtw and a increased trend in GCS( P 〉0.05 ). Compared with LVN group, the value of GLS,IUR were further decreased, Ptw, PTV, PUTV were further increased, GRS was reduced , and tPUTV was prolonged in LVR group( P 〈 0.05 ). Conclusion STE can sensitively reflect the changes of left ventricle by evaluating the left ventricular two - dimensional strain and torsion in patients with EH.
