腹腔镜与开腹手术治疗小儿急性阑尾炎的对比分析

目的对腹腔镜与开腹手术治疗小儿急性阑尾炎的疗效进行对比。方法 2008年7月至2011年8月重庆市南川区人民医院明确诊断的急性阑尾炎患儿66例根据家属意愿,分为腹腔镜组和开腹手术组,对两组的临床疗效进行对比。结果腹腔镜组手术时间、术后进食时间、住院时间分别为(42.4±17.5)min、(15.5±4.6)h、(5.3±1.2)d,均短于开腹手术组的(50.8±18.7)min、(19.7±5.4)h、(7.8±2.5)d;腹腔镜组术中出血为(6.8±4.5)mL,明显少于开腹手术组的(10.5±3.9)mL,差异有统计学意义(P<0.05);腹腔镜组术后切口感染、腹腔脓肿及粘连性肠梗阻发生率明显低于开腹手术组,差异有统计学意义(P<0.05)。结论腹腔镜治疗小儿急性阑尾炎可缩短手术时间、下床时间、术后进食时间、住院时间,且创伤小、术后并发症少,可明显减少患儿痛苦,适合在临床推广应用。

Comparative analysis of laparoscope and open surgery on treating children acute appendicitis

Objective To compare the effects of laparoscope and open surgery on treating children acute appendicitis. Methods 66 children with acute appendicitis in the hospital from July 2008 to August 2011,were divided into laparoscope group and open surgery group according to the parent＇s will, with the comparison of the clinical effects. Results The operation time,post operation meal time and hospital stays of laparoscope group were （42.4±17.5） rain, （ 15.5±4.6） h, （5.3 ± 1.2） d, which were shorter than those of open surgery group （ 50.8 ± 18.7） min, （19.7±5.4）b,and （7.8±2.5）d. The intra operative bleeding of laparoscope group was （6.8±4.5）mL,which was less than that of open surgery group（10.5 ± 3.9）mL, with significant difference（P〈0.05）. The incidence rates of wound infection,abdominal abscess and adhesive intestinal obstruction in laparoscope group were lower than those of open surgery one,with significant difference（P〈0.05）. Conclusion The laparoscope application on treating children with acute appendicitis will short operation time,out of bed activity time,post operation meal time,and hospital stay time with little trauma and less complication,which deserves to be spread in clinic.
