低分子肝素雾化吸入在慢性肺心病治疗中的作用

目的：探讨低分子肝素钙雾化吸入对慢性肺源性心脏病的治疗价值。方法：89例患者随机分为治疗组与对照组，对照组给予常规治疗，治疗组在常规治疗的基础上应用低分子肝素钙雾化吸入，100IU／kg，每12小时1次，疗程10天。治疗前后监测肺平均动脉压、动脉血气分析、血凝及纤溶指标，治疗后评估疗效。结果：临床疗效治疗组优于对照组（P〈0．05），治疗组肺平均动脉压下降优于对照组（P〈0．05）；动脉血氧分压升高治疗组优于对照组（P〈0．05）；血凝及纤溶指标有变化、但未引起出血现象。治疗组血浆凝血酶原时间、凝血酶时间在治疗前后比较差异有显著性（P〈0．05），治疗后治疗组与对照组比较，血浆凝血酶原时间、凝血酶时间变化差异有显著性（P〈0．05）。结论：低分子肝素钙雾化吸入可改善慢性肺源性心脏病的肺动脉压，改善肺通气功能。

The role of low molecular weight heparin calcium aerosol inhalation in chronic cor pulmonale

Objective:To investigate the therapeutic value of low molecular weight heparin calcium(LMWH Ca) aerosol inhalation in chronic cor pulmonale.Methods:89 patients were randomly divided into two groups,the treatment group and control group.The treatment group was added with LMWH Ca aerosol inhalation 100 IU/kg q12h besides routine treatment,which was used in the control group.The course of treatment was 10 days.The pulmonary average arterial pressure,the arterial blood-gas analysis and the indices of blood coagulation and fibrinolysis were detected before and after treatment,and their therapeutic effects were evaluated after treatment.Results:The treatment group was better than the control group in the clinical effect(P<0.05),declining pulmonary average arterial pressure(P<0.05) and improving the oxygen partial pressure of blood(P<0.05).There were changes in the indices of blood coagulation and fibrinolysis,but hemorrhagic phenomenon was not found.The differences of prothrombin time and thrombin time in the treatment group before and after treatment were significant(P<0.05),and so were those between the treatment group and control group after treatment(P<0.05).Conclusion:LMWH Ca aerosol inhalation for chronic cor pulmonale could improve the clinical effect,decline pulmonary arterial pressure and improve ventilate function.
