| 摘 要： | 目的探讨恶性实体瘤化学治疗后骨髓抑制的脊柱磁共振影像学表现。方法搜集脊柱增强MRI及同位素骨扫描检查除外脊柱转移瘤且临床排除其他原发骨髓疾病的恶性实体瘤患者54例，根据在MR前是否接受过化疗及是否发生急、慢性骨髓抑制将病例分为骨髓抑制急性组、慢性组，化疗后无骨髓抑制组及未化疗组共4组，比较MR表现异同。结果急性组5例，慢性组24例，化疗后无骨髓抑制组7例，未化疗组18例。慢性组及化疗后无骨髓抑制组的平均椎体／脑脊液信号强度比值高于未化疗组（P〈005），急性组中4例信号均匀减低；慢性组及无骨髓抑制组平均椎体信号强度标准差高于未化疗组（P〈0.05）。4组中局灶性脂肪沉积发生率无统计学差异。结论恶性实体瘤化学治疗后骨髓抑制患者脊柱椎体T1信号发生改变，慢性期不均匀性升高，急性期呈均匀减低趋势；局灶性脂肪沉积不是骨髓抑制的特异性征象。
