颌面颈部血管滤泡性淋巴组织增生的影像学表现与病理分析

摘    要:目的:分析颌面颈部血管滤泡性淋巴组织增生的影像学表现,并与病理结果进行对照分析,旨在提高对该病的术前诊断。方法:回顾性分析12例经病理证实的颌面颈部血管滤泡性淋巴组织增生的影像学表现。结果:12例颌面颈部血管滤泡性淋巴组织增生中,左颈部6例,其中左腮腺区1例,左颌下区2例,左颞浅部1例;右颈部4例;双颈部2例。12例均行超声检查显示为颈部淋巴结肿大或无痛性包块;7例行CT检查、1例行CT+MRI+MRA检查,1例行MRI+MRA+DSA检查;CT、MRI、MRA表现为单发或多发边缘光滑、清晰的软组织团块影,增强后所有病灶不同程度强化,大部分强化近血管程度,相邻处气管及血管受压、移位。1例行DSA表现为颈部块影,实质期明显着色。病理为透明血管型9例,局限浆细胞型1例,混合型2例。结论:颌面颈部血管滤泡性淋巴组织增生的影像学表现具有一定的特征,认识其表现有利于提高术前诊断。

Imaging findings and pathologic analysis of the maxillofacial and neck angiofollicular lymphonode hyperblastosis

Abstract:Objective:To analyze the imaging features of the maxillofacial and neck angiofollicular lymphonode hyperblastosis (AFH) and the correlation with histopathologic findings to improve the preoperative diagnosis. Methods: The imaging data of 12 patients who has been confirmed with AFH pathologically were retrospectively analyzed. Results: In six patients, AFH was located in the left neck 1 in the left parotid gland, 2 in the left submandibular area, 1 in the left superficial temporal division. Four lesions were located in the right neck, and the other two were in the the double neck. Ultrasound examination revealed a cervical lymph node enlargement or painless mass in all patients. Seven patients underwent CT examination, one with routine CT + MR + MRA, and another with a routine MR + MRA + DSA examination. The lesions appeared single or multiple well - defined soft tissue mass with the different degree of enhancement. The enhancement of the most lesions was similar to the blood vessels. The near respiratory and vascular compression was revealed. DSA revealed the mass with significantly coloring in the neck. The pathological findings showed the hyaline vascular type in 9 patients, plasma cell type in 1 patient, hybrid type in two patients. Conclusion: The imaging findings of maxillofacial and neck AFH is characteristic in certain degree, which is helpful to improve the preoperative diagnosis.
