乳腺外科围手术期抗菌药物合理应用的研究

目的通过对乳腺外科围手术期抗菌药物合理应用的干预对照研究，探索促进我院I类切口围手术期合理使用抗菌药物的干预模式，促进临床合理用药。方法选取我院乳腺外科2011年1月1日至2011年4月30日出院的全部手术病例为于预前组，2011年7月1日至2011年10月31日出院的乳腺外科全部手术病例为干预后组，比较干预前后预防性使用抗菌药物的合理性。结果干预后平均抗菌药物费用由于预前的256．97元下降至96．59元，干预后未使用预防性抗菌药物比例由干预前的0．00％上升至12．84％，干预后第一代头孢菌素的使用率由干预前的3．00％上升至13．67％。实施综合干预措施后，预防性使用抗菌药物的合理性明显改善。结论本研究所采用的综合干预措施具有可行性和有效性，促进了抗菌药物的安全、有效、经济、合理的使用。

A Research on Rational Use of Antibiotics during Perioperative Period of Breast Operation

Objective Through intervention comparison research on rational application of antibiotics during preoperative period in breast surgery, this paper discusses intervention mode of rational application of antibiotics at type during I incision during perioperative period for promoting rational medication in clinical. Methods All operation cases discharged from Jan 1, 2011 to April 30, 2011 are selected as pre-intervention group in breast surgery in our hospital; all the operation cases discharged from July 1, 2011 to Oct 31, 2011 are selected as post-intervention group; it compares the rationality of antibiotics medication in two groups. Results After intervention, the average expenses of antibiotics decreases from 256.97 yuan before in- tervention to 96.59 yuan; the proportion of no use preservative antibiotics increased from 0.00% to 12.84% ; the utilization rate of the first-generation cephalosporins increased from 3.00% to 13.67%. After the implementation of the integrated interventions, the rationality of preservative use of an- tibiotics is significantly improved. Condluions The integrated intervention methods used in this study are proved practicable and effective which contributes to promote the safe, effective, economic and rational use of antibiotics.
