肾血管平滑肌脂肪瘤10例临床病理与免疫组化研究

目的 探讨肾血管平滑肌脂肪瘤的临床病理学特点、并通过免疫组化方法探讨其组织来源。方法 收集10例肾血管平滑肌脂肪瘤的临床资料,常规石蜡切片,进行免疫组化染色。结果 大体上5例为境界清楚的包块,5例与肾组织有粘连。镜下观察:肿瘤由血管、脂肪、平滑肌细胞混合组成,3种成分构成比例不同。1例有髓外造血。免疫组化表达HMB45、SMA、CD34。讨论 肾血管平滑肌脂肪瘤形态多样,影像学检查有助于术前诊断。HMB45、SMA、CD34的表达提示其来源于具有多向分化潜能的细胞。

Clinicopathologic and Immunohistochemical Study on Renal Angiomyolipoma

Objective To investigate the clinicopathologic characteristics and the origin of renal angiomyolipoma. Methods 10 cases of renal angiomyolipoma were retrieved,using immunohistochemistry ABC method to test the expression level of several antibodies. Results 5 tumors were well-circumscribed,5 were not easy to separate from kidney. Histologically they were composed of three components:thick-walled blood vessels,epithelioid smooth muscle cells and fat cells. All of 10 had high rate of positive expression of HMB45,SMA,CD34. Conclusion Renal angiomyolipoma shows a benign course with no recurrences,X-ray will help to make diagnosis before surgery. The expression of HMB45,SMA,CD34 reveals that renal angiomyolipoma comes from cells which have multiple potential ability to differentiate.
