ER、PR、PCN及c-erbB-2在子宫内膜腺癌中的表达及与分化水平的关系

摘    要:目的:研究女性激素、增殖细胞核抗原(PCNA)、原癌基因c-erbB-2蛋白在子宫内膜增生过长和子宫内膜腺癌发病中的作用及对癌细胞分化的影响,为临床内分泌治疗提供理论依据。方法:采用免疫组织化学SP法,对手术切除和诊刮的37例子宫内膜腺癌和20例子宫内膜增生过长标本进行ER、PR、PCNA和c-erbB-2含量检测。并结合组织学类型和分化类型进行分析。结果:内膜腺癌组织中ER、PR的含量低于增生过长内膜组织(P<0.01),与肿瘤的组织学类型和分化有关。PCNA和c-erbB-2在内膜腺癌中的含量显著高于增生过长内膜组织(P<0.01),与分化程度呈负相关。与ER、PR的含量无明显相关性。结论:ER在子宫内膜增生过长和内膜腺癌的发病中起主要作用,PCNA和c-erbB-2的过表达可能与细胞的异常生长和分化有关。

The Expressions of Estrogen Receptor,Progesterone Receptor, Proliferating Cell Nuclear Antigen and C-erbB-2 and Their Correlation with the Differentiation Level in Endometrial Adenocarcinoma
Abstract:Objective To explore the effects of female sex steroid hormones, proliferating cell nuclear antigen(PCNA), and c-erbB-2 on the development of endometrial adenocarcinoma and hyperplasia and on the tumor cell differentiation , thereby seeking theoretical basis for the clinical endocrine therapy.Methods:The expressions of estrogen receptor(ER), progesterone receptor (PR),PCNA,c-erbB-2 were determined by immunohistochemical SP method in specimens of endometrial adenocarcinoma (37 cases) and endometrial hyperplasia (20 cases) obtained from women undergoing hysterectomy and biopsy diagnostic curettage respectively, and meanwhile the expressions were analyzed in the case of histologic type and differentiation level.Results:The expression contents of ER and PR were significantly lower in endometrial adenocarcinoma than in the hyperplasia (P<0.01). They were related to the histologic type and tumor cell differentiation level. The expression of PCNA and c-erbB-2 was significantly higher in the adenocarcinoma of endometrium than in the endometrial hyperplasia (P<0.01), but they were inversely related to the differentiation level, and there was no clear correlation with the expression of ER and PR.Conclusion:ER may play an important role in the pathogenesis of endometrial adenocarcinoma. The over-expression of PCNA and c-erbB-2 could be correlated with the abnormal cell growth and differentiation.
