| 摘 要： | 目的研究糖尿病大鼠肾脏损伤时nephrin、podocin的表达变化以及应用来氟米特和氯沙坦对其表达的影响。方法将48只清洁级雄性Wistar大鼠随机分为4组:单肾切除对照组(N组),模型组(D组),来氟米特治疗组(LEF组),氯沙坦治疗组(L组)。分别于糖尿病成模后8、12周处死,观察24h尿微量白蛋白排泄量、尿素氮(BUN)、肌酐(SCr)、肾脏病理组织学改变,应用免疫组织化学方法检测肾脏皮质nephrin、podocin蛋白的表达水平。结果与N组比较,D组出现①进行性微量白蛋白尿,BUN、SCr增高(P<0.05);②肾组织内nephrin、podocin表达下调(P<0.05)。与D组比较,各干预组(LEF组、L组)①24h尿微量白蛋白排泄量、BUN、SCr均降低(P<0.05);②nephrin、podoin表达增加(P<0.05)。两组药物之间比较差异无统计学意义(P均>0.05)。结论来氟米特和氯沙坦均对糖尿病大鼠肾脏损害有保护作用,其机制可能是通过增加nephrin、podocin表达而实现。
