| 摘 要： | 目的家庭医生制度是国家新医改的重要内容,也是上海市即将要全面实施的综合改革目标。笔者根据多年的研究实践,对家庭医生制度进行首次界定,回顾了国际家庭医生服务的发展趋势和意义。同时,对家庭医生制度内涵和外延进行了探讨,提出研究家庭医生制度＂54321＂方案：5个确定,即确定家庭医生资质,确定供方服务内容,确定需方利用程度,确定服务项目费用;4方付费,就是政府支付内容,基本医疗保险支付,社会支付,个人支付;3方评价,就是供方评价,需方评价,第三方评价;最后初步制定了家庭医生制度发展的战略,就是加强领导,综合管理,政策配套,协调发展;改革体制,引入竞争,积极创新,稳步发展;心系社区居民,完善服务、深化服务、可持续服务;制定规划,多种模式,多种层次,循序渐进,积极培养家庭医学人才;加强管理,保证质量,良性运转,健康发展。
