超声弹性成像指导泼尼松治疗亚急性甲状腺炎的初步应用

目的探讨超声弹性成像指导泼尼松治疗亚急性甲状腺炎（subacute thyroiditis,SAT）的临床意义。方法对临床疑似SAT的46例患者行超声引导下细针穿刺活检，将确诊为SAT的39例患者随机分成两组行泼尼松治疗，治疗组20例（30个病灶）用超声弹性成像观察疗效并指导用药，对照组19例（28个病灶）根据临床症状指导用药，并在停药后跟踪观察12个月。结果SAT患者患侧甲状腺呈轻中度肿大，部分呈双侧发病，两组病例治疗过程中均未见明显不良反应，治疗组治愈率96.77%，对照组治愈率71.43%，两组比较差异有统计学意义（P＜0.05）；而两组泼尼松的疗程、总用药量差异无统计学意义（P＞0.05）。结论SAT超声弹性图像具有一定的特征性表现，超声弹性成像可指导泼尼松治疗SAT。
