儿童颅内高分级室管膜瘤的术后全中枢神经系统放疗（附7例报告）

目的：探讨儿童颅内高分级室管膜瘤的术后放射治疗。方法：1991年1月至1998年2月，收治的7例儿童颅内高分级室管膜瘤行术后全中枢神经系统放疗，观察其疗效及毒副反应，并结合献资料进行讨论。结果：本组1、3和5年生存率分别为100％、71．4％和47．6％，死亡3例，其中2例死于局部复发，占死亡患儿的66．7％（2／3），1例死于局部残留病灶进展。本组未观察到放射脊髓病。结论：对儿童高分级室管膜瘤的术后放射治疗，宜选用全中枢神经系统放疗技术。局部复发或局部残留病灶进展是治疗失败的主要原因。

Postoperative craniospinal irradiation of pediatric intracranial high - grade ependymoma( report of seven cases)

Objective: To investigate the efficacy of postoperative craniospinal irradiaton for pediatricintra-cranial high-grade ependymoma. Methods: From January 1991 to February 1998, 7 children with high- grade intracranial ependymoma were treated with craniospinal field in our department. The efficacy and toxicity were observed and analysed with the literature review. Results: 1, 3, 5-year survival rate in the group were 100% , 71.4% and 47.6% respectively. Three cases were died, of which 2 died of local recurrence and 1 of local residual progression. During the follow-up period, no myelopathy was observerved.Conclusion: Whole craniospinal irradiation is preferable for pediatric intracranial high-grade ependymoma. Local recurrence or local residual progression are the main cause of treatment failure.
