铸造上颌板后堤区的板状设计和网状设计的临床应用

摘    要:目的比较铸造上颌腭板基托后堤区两种不同设计的临床效果。方法采用刮除石膏的方法制作后堤区,在上颌基托硬腭区采取金属板设计,牙槽基采用网状设计,206个病例后堤区随机分别采用网状和板状设计。在戴牙4周后对上颌义齿的固位效果进行评价。对由后堤区固位不佳引起固位不良的网状设计义齿重衬后再次评价。结果103例板状设计中,固位一般的义齿7例,固位差的义齿5例。由后堤区封闭不佳造成的固位一般的5例,固位差的3例;俱重新制作后,固位佳的义齿6例,一般2例。103例厚堤区设计为金属网加强塑料者,固位一般者9例,固位差者2例。由后堤区封闭不良引起者,固位一般义齿7例,固位差的义齿2例。经后堤区重衬后,固位佳的8例,一般的1例。结论上颌全口义齿铸造颚板后堤区的网状设计较板状设计,既有常规设计的优点,又能重衬,提高了义齿的成功率。

Abstract:Objective To evaluate the clinical effect of the net and plate design of casting baseplate of postdam area in complete denture. Methods Scraping off gypsum to form postdam area,adoping plateau design on hard plate area of maxillary baseplate, net design on the alvolar creat. Patients were randomly divided into two groups: net and plate design groups. Four weeks later after putting on denture, the author evaluated the retention of maxillary denture. If retention was not enough because of poor postdam area seal, another evaluation would adopt for dentures of net design in postdam area after relining postdam area. Results There were 103 patients adopted plate design, emong them 91 patients had excel- lent retention, 7 dentures general retention, and 5 bad retention. 5 dentures with general retention and 3 with bad retention were caused by deficient seal of postdam area . New dentures had to be done. Among the net shape designs, 92 patients had excellent retention, 9 dentures general retention, and 2 bad retention. 7 dentures with general retention and 2 with bad retention were caused by deficient seal of postdam area. After relining postdam area on above 9 dentures, 8 dentures received excellent retention and 1 received general retention. Conclusion Anasist plate design, net shape design in the maxillary complete denture of casting metal base plate in postdam area have the merit of conventional design and improve the aeeucessment of complete denture.
