正常人三尖瓣环组织多普勒速度成像

摘    要:目的:用超声组织多普勒技术研究三尖瓣环的运动特点,为三尖瓣环组织多普勒检查的临床应用提供依据.方法:应用sequoia-512彩色电脑声像仪,探头型号3V2c,H3.5MHz,选用胸骨旁四腔切面(PS4cV)、心尖四腔切面(AP-4cV)观察60名健康志愿者三尖瓣环前瓣(AAP)及隔瓣附着点(sAP),心尖右心两腔切面(AP-RH-2cV)观察三尖瓣环后瓣附着点(PAP).常规测量三尖瓣环上述五个位点的收缩期峰值速度νs舒张早期峰值速度vdR。舒张晚期峰值速度νdl,计算各点的νdR/νdL值.采用统计软件SPSS11.0进行多组均数比较的方差分析及均数的多重比较.结果:三尖瓣环五个附着点收缩期运动速度νs、舒张早期运动速度νdR、舒张晚期运动速度νdL。总体上有显著差异,νdR/νdL值总体上无显著差异.结论:三尖瓣环的运动存在不对称性,应用组织多普勒技术测量三尖瓣环的运动参数进行临床评价时要注意选择适当的观测点.

Doppler tissue velocity imaging of tricuspid annulus in healthy people
Abstract:AIM: To study the characteristics of tricuspid annulus movement for better clinical application of tissue Doppler imaging (DTI) of tricuspid annulus. METHODS: Doppler tissue velocity mode was used and the tricuspid annulus movements of anterior, septal and posterior attachment points in 60 healthy people were detected from apical four chamber view (AP-4CV), parasternal four chamber view (PS-4CV) and apical right heart two chamber view (AP-RH-2CV). The peak systolic flow velocity v s, peak diastolic early period flow velocity v dR and peak diastolic late period flow velocity v dL in the five points of tricuspid annulus were recorded and the values of v dR /v dL in the five points were calculated. The data were analyzed with analysis of variance (ANOVA) and multiple comparisons by SPSS 11.0 . RESULTS: There was significant difference in the velocity of v s, v dR and v dL in the five points of tricuspid annulus in gross (v s, F=53.40, P<0.05; v dR , F=15.62, P< 0.05 ; v dL , F=16.91, P<0.05), but there was no significant difference in the values of v dR /v dL in the five points of tricuspid annulus in gross (v dR /v dL , F=2.41, P>0.05). CONCLUSION: There is asymmetry in the tricuspid annulus movement, and it is important to choose the appropriate point in applying tissue Doppler imaging technique to study the movement of tricuspid annulus.
