肝静脉血流多普勒频谱变化评估肺动脉高压

摘    要:用彩色多普勒超声心动图观察了30例正常人和右心导管测压的28例先天性心脏病人的肝静脉血流(HVF)频谱。结果显示肺动脉高压(PH)病人HVF频谱测值具有特征性变化,HVF频谱改变评估PH可以弥补其它方法之不足或结合其它方法提高估测肺动脉压的准确性。

Abstract:For seeking a new approach to evaluate pulmonary hypertension (PH) by the changes ofhepatic vein flow (HVF) spectrum, we examined HVF velocity wave-form in 30 nomal subjects and 28patients of congenital heart disease undergoing diagnostic right cardiac catheterization by color Dopplerechocardiography and found the characteristic changes in HVF spectrum of patients with PH. The bestcorrelation was acquired between (aVm vVm ) / (sVm dVm ) or (aVI vVI ) / (sVI dVI ) and meanpulmonary artery pressure (r=0. 7057,r= 0. 6509,P<0. 01 respectively). Evaluating PH with (aVm vVm)/(sVm dVm) or (aVI vVI)/(sVI dVI) resulted in better sensitivity and specificity. We concluded that the changes of HVF spectrum is a new useful method in evaluating PH and could counteractthe weakness of the other ways or assess the pulmonary artery pressure well as combined with the others
