芪参益气滴丸对老年心力衰竭患者心功能及淋巴细胞亚群的影响

目的：研究芪参益气滴丸对老年慢性心力衰竭患者心脏功能及淋巴细胞亚群的影响。方法：入选80例60岁以上、纽约心功能分级（NYHA分级）为Ⅱ～Ⅳ级的老年慢性心衰患者，将其按照随机数字表法分为心衰试验组和心衰对照组各40例。对照组采用常规治疗，试验组在常规治疗的基础上加用芪参益气滴丸治疗3个月。另选40例60岁以上健康老年人作为正常组，分别在用药前后测定各组患者的淋巴细胞亚群、血浆B型利钠肽（BNP）和心功能超声检查各项参数的改善情况。结果：（1）治疗前与正常组相比，心衰对照组和试验组患者的血浆BNP，抑制性T淋巴细胞（CD8）均增高。成熟T淋巴细胞（CD3）、辅助性T淋巴细胞（CD4）、CD4／CD8、心功能参数均降低，差异均有统计学意义（P〈0．05或P〈0．01），心衰对照组与试验组两组间比较各项指标差异无统计意义。（2）治疗后心衰试验组与对照组相比，血浆CD3、CD4、CD4／CD8显著增高，心功能参数改善明显，差异均有统计学意义（P〈O．05），血浆BNP和CD8显著降低（P〈0．05或P〈0．01）。结论：芪参益气滴丸能够改善老年心衰患者淋巴细胞比例失衡，进而改善心功能，为老年心衰患者提供了新的辅助治疗途径。

Effects of Qishen Yiqi Diwan on Cardiac Function in Aged Patients with Chronic Heart Failure

Objective： To investigate the effect of Qishen Yiqi Diwan on cardiac function and lymphocyte subsets in aged patients with chronic heart failure （ CHF ） .Method： CHF （ NYHA classification： II - 1V ） patients older than 60 years were according to the randomly divided into CHF trial group and CHF control group, 40 cases in each group.The control group was given conventional treatment, the experimental group was given Qishenyiqi pills for 3 months based on routine treatment.Another 40 healthy individuals older than 60 years were selected as normal control group.lymphocyte subsets, plasma brain natrium peptide （ BNP ） and uhrasonography parameters of cardiac function were measured at before therapy, after treatment.Result： （ 1 ） Compared with normal group before treatment, the levels of BNP, CD8 T ceils of CHF trim group and CHF control group were increased and the levels of CD3, CD4 T cells, CD4/CD8 ration and ultrasonography parameters of cardiac function were decreased, the differences were statistically significant （ P〈0.05 - P〈0.01 ） .These parameters were similar between CHF intervention group and CHF control group. （ 2 ） After treatment, the levels of CD3, CD4 T cells, CD4/CD8 ration, and ultrasonography parameters of cardiac function were significantly in creased, the difference was statistically significant （ P〈0.05 ） .While the levels of CD8 and BNP were significantly decreased in CHF intervention group compared with CHF control group （ P〈0.05 or P〈0.01 ） .Conclusion： Qishen Yiqi Diwan may regulate the quantity and proportion of lymphocyte subsets and improve cardiac function in aged patients with CHF, indicating that it may be a new assistant treatment strategy for aged CHF paitents.
