自体组织“桥”式修复术治疗阴道前后壁脱垂38例报告

目的：对传统的阴道前后壁修补术进行改进,提高阴道脱垂的手术疗效。方法：38例盆腔器官阴道脱垂患者全部为阴道前后壁同时脱垂,其中32例同时合并Ⅱ度以上子宫脱垂。在传统阴道前后壁修补术基础上,利用患者自体组织对直肠阴道筋膜、膀胱阴道筋膜进行加固,同时辅以单极电凝热透法电凝＂桥＂体表面的黏膜组织。结果：38例均治愈。手术时间平均为41.5 min,出血量平均为38.4 ml。未发生尿道、膀胱、直肠损伤及血肿。术后拔除导尿管均能自解小便,残余尿均〈100 ml。结论：阴道前、后壁＂桥＂式缝合术治疗阴道前后壁脱垂组织相容性好,价格低廉,容易操作,易被患者接受,容易推广。

Autogenous Tissue Vaginal Anterior-posterior Repair 38 Cases of Vaginal Prolapse

Objective： To improve vaginal anterior-posterior repair and operative effect of vaginal prolapse.Method： 38 patients with vaginal anterior-posterior prolapse, and 32 eases among them combined with over degree Ⅱ uterine prolapse were analyzed.Based on traditionally vaginal anterior-posterior repair, the operation strengthens rectovaginal fascia and vesieovaginal fascia used by autologous tissue, at the same time with monopolar electrecoagulation heat through electric coagulation ＂bridge＂ body surface of mucosa.Result： 38 patients were cured.The operation time was 41.5 min, and average blood loss was 38.4 ml.The urethra, bladder and rectum were not damaged and there were not hematoma formation.The patients could urinate by themselves under removal of the catheter and average residual urine〈100 ml.Conelusion： Vaginal anterior-posterior bridge repair has some advantage in the treatment of vaginal anterior-posterior prolapse such as good compatibility, low price, easy operation, easy acceptance by patients and easy extension.
