胃癌根治术引起脾脏损伤的临床研究

目的：调查研究胃癌根治术的过程中引起脾脏损伤的原因以及预防措施。方法：对37例胃癌根治术引起脾脏损伤患者的临床资料进行回顾性的分析。结果：在进行调查的37例脾脏损伤患者中有29例进行修补手术,使用明胶海绵和医用胶进行黏合的患者有18例,使用止血纱布联合医用胶进行黏合的有11例,8例患者发生脾出血后进行脾脏切除手术或者结扎手术;在37例患者中有29例保脾成功,且成功率达到78.4%,所有的患者都在治愈后出院。其中,有17例患者在进行远端胃大切手术时出现了脾脏受损现象,11例患者在进行近端胃大切手术时出现了脾脏受损现象,9例患者在进行胃全切手术中发生脾脏受损。结论：不断提高保护脾脏的意识,不断更新并及时改善手术过程中技术的操作,使用有效的止血手段,可以有效预防在胃癌根治手术中引发脾脏受损。

Clinical Study of Splenic Injury Caused by Radical Surgery for Gastric Cancer

Objective： To research the causes in the process of gastric cancer radical cause spleen damage and prevention measures.Method： The clinical data of 37 patients with spleen injury which caused by radical surgery of gastric cancer were analyzed retrospectively.Result： In 37 patients there were 29 cases of splenic salvage success, and the success rate of 78.4%, all patients were discharged in cure.Among them 17 patients had a damaged spleen phenomenon in the distal subtotal resection operation, 11 patients in the proximal stomach cutting operation when a damaged spleen, 9 patients in the gastric resection operation of a damaged spleen.Conclusion： The research result shows that it is effective to prevent spleen damage for gastric cancer radical surgery by means of improving operation skills and hemostasis efficiency on the basis of protection awareness of the spleen.
