颈椎椎体间异体骨螺纹融合支架的设计和临床应用

摘    要:目的 探讨以异体骨螺纹融合支架(allograftthreaded fusion cage,ATFC) 做颈椎椎体间融合的临床效果。方法 自1996 年4 月用自行研制的ATFC 治疗颈椎间盘疾病18 例,21 个椎节。术后观察临床效果及手术并发症,并做X 线征评价。结果 手术18 例,获随访15 例,平均随访15 个月。临床疗效满意,无神经并发症发生。ATFC 无移位及骨折。术后3 个月动态摄X 线片,除1 个椎节未融合外,其他均获骨性融合。手术椎间隙无高度丢失或成角畸形。结论 作者认为同传统颈椎前路减压椎间融合术相比,该手术优点在于:(1) 术中无需再取自体髂骨植骨,手术创伤小,时间短;(2) 椎间融合率高,可以达到近似自体髂骨植骨的融合率;(3) 支架置入牢靠,椎间稳定性好;(4)能有效维持椎间隙高度,减少椎间隙狭窄带来的并发症

Abstract:Objective To investigate the result of using allograft threaded fusion cage(ATFC) for cervical interbody fusion. Methods Eighteen cases of cervical disc disease were treated with anterior decompression combined with ATFC from April 1996 to May 1997. The clinical result, radiographic outcome and the operation complication were observed. Results Fifteen cases had excellent resutls with an average of 15 months follow up.No neurologic complications were seen, no fracture and dislodgement were observed in ATFC. The interspace was maintained. All but one segment had solid fusion. Conclusion In comparison with the conventional methods, the ATFC has following advantages: (1) a simple procedure with less operative trauma and short operation time, without the necessity of harvesting autograft.(2) the fusion rate was similar to that of autograft. (3) strong fixation ensured intervertebral stability over the operated segments. (4) the interspace was maintained, eliminating the neurologic complications of a narrow interspace and anterior angulation.
