经皮神经电刺激预防失神经肌萎缩的实验研究

摘    要:目的:观察经皮神经电刺激(TENS)在减轻失神经肌肉萎缩和提高神经端侧缝合侧枝萌出率中的作用。方法:18只成年新西兰白兔,随机分为A、B、C3组,切断双侧腓神经后与外膜开窗的胫神经端侧缝合。左后肢为实验侧,给予TENS,持续6周。右后肢为对照侧,不给电刺激。分别于术后3、6、16周取材,进行大体观察、神经肌肉组织学、神经电生理、肌湿重和透射电镜检查。结果:各组实验侧胫前肌湿重和肌纤维截面积,腓神经有髓纤维数、传导速度和振幅均高于对照侧(P<0.05),A组两侧肌湿重及C组腓神经潜伏期差异无显著性(P>0.05)。结论:TENS在减轻失神经肌肉萎缩和提高神经端侧缝合侧枝萌出率方面有积极作用。

Abstract:Objec tive:To study the effect of transcutaneous electrical nerve stimulation(TENS)on preventing the denervated muscle atrophy and peripheral nerve regeneration afte r end-to-side neurorrhaphy in rabbit model.Method:Eighteen five-month-old New Z ealand rabbits were randomly divided into three groups.The peroneal nerve was tr ansected and then its distal end was sutured to the tibial nerve which had been windowed by means of end-to-side attachment at2cm above the knee in both hind li mbs.The left hindlimb served as the experimental side that accepted TENS and the right hindlimb as the control side.The toe-extension reflex,anterior tibial mus cle wet weight,histological and electrophysiological tests were examined at3,6an d16weeks postoperatively.Result:All the above mentioned indices in the experime ntal side were much better than those in the control side(P<0.05or P<0.01)exc ept the wet weight of A group and the latency of C group(P>0.05).Conclusion:TEN S can effectively prevent denervated muscle from atrophy and promote the rate of peripheral nerve collateral sprouting after end-to-side neurorrhaphy to a certa in extent.
