老年女性2型糖尿病患者心率变异性与24小时平均脉压及心律失常的关系

目的 探讨老年女性2型糖尿病患者心率变异性(HRV)与24h平均脉压(APP)及心律失常的关系.方法 选择36例老年女性2型糖尿病患者(糖屎病组)和30例健康体检者或非糖尿病及心血管疾病的住院患者(对照组).两组受检者进行全程24h动态心电图和24h动态血压检查,进行HRV时域指标(正常R-R间期的标准差、每5minR-R间期平均值的标准差、每5min R-R间期标准差的平均值、相邻R-R间期差值的均方根、正常正邻R-R间期差值＞50ms的计数占总R-R问期数的百分比)以及APP计算并分析.结果 糖尿病组和对照组的HRV时域指标值、室上性期前收缩和室性期前收缩总数及APP＞50mm Hg者所占比例比较差别均有统计学意义(P＜0.05).结论 老年2型糖尿病患者心率变异性越低,平均脉压越高,心律失常的发生率就越高.

Heart Rate Variation and 24-Hour Average Pulse Pressure and Arrhythmia in the Aged Women with Type 2 Diabetes

Objective To study the relations among heart rate variation (HRV),24 h average pulse pressure (APP) and arrhythmia in the aged female patients with type 2 diabetes.Methods Totally group of 36 aged female patients with type 2 diabetes as diabetic group and 30 subjects who are health persons or non diabetic and non-cardiovascular disease inpatients as control group were selected.The subjects in both groups were given a 24-h dynamic electrocardiography and 24-h development blood pressure examination.Their HRV time domain indexes (SDNN,SDANN,SDNN Index,rMSSD,PNN50) and APP were calculated and analyzed.Results The HRV time domain indexes SDNN,SDANN,SDNN,,rMSSD and PNN50 were lower in the diabetic group than in the control group,while the total numbers of superventricular and ventricular extrasystole,and the percentage of APP>50mm Hg higher (P<0.05).And the prevalence of arrhythmia in the diabetics was obviously higher than that of the normal subjects.Conclusion The lower the HRV and the higher the average pulse pressure,the higher the prevalence of arrhythmia in aged female patients with type 2 diabetes.
