p27在人脑星形细胞肿瘤中的表达及意义

目的 :了解p2 7蛋白在人脑星形细胞肿瘤中的表达 ,探讨 p2 7LI(标记指数 )在人脑星形细胞肿瘤中的临床意义。方法 :收集自 1992～ 1997年本院神经外科手术治疗的原发性星形细胞肿瘤患者 73例的资料 ,包括病理学分级及术后生存时间。使用S -P免疫组化方法检测手术切除标本石蜡块的p2 7抗原并计算其LI。对比研究了不同组织级别中p2 7蛋白的表达水平 ,使用Kaplan -Meier法计算生存率并采用logrank(时序 )检验判断p2 7LI在星形细胞肿瘤患者中的可能的预后价值。结果 :p2 7LI范围 3 .15 %～ 86.4% ,均值为 2 4.7%± 2 2 .0 % ;在组织学分级WHOⅡ、Ⅲ、Ⅳ中分别为 3 8.6%± 19.8%、2 7.8%± 2 4.7%、12 .1%± 13 .3 % ,三组间有显著差异 (P <0 .0 1)。p2 7LI与病理级别呈明显负相关。Kaplan -Meier生存概率曲线显示 p2 7LI的高低对患者的预后有显著的统计学意义。 结论 :p2 7LI随着各病理级别增高而下降 ;p2 7LI下降提示预后较差。在同一病理级别 (GradeⅢ、Ⅳ )中 ,p2 7LI不同 (≥或 <17 4% ) ,其预后有显著性差异。

The Expression of p27/Kipl in Human Astrocytic Tumor and It''''s Clinic Value

Objective:To explore the expression of p27 in human astrocytic tumors and to find the clinic value of p27 labeling index obtained using p27 monoclonal antibody.Methods:Data of 73 patients with astrocytic tumors were collected enrolled in affiliated hospital of Nantong Medical College from 1992 to 1997. Relevant information included patient age, gender, histologic grade, date of death, status at the time of last communication (dead and cause of death or alive). p27 was determined with immunohistochemistry using monoclonal antibody. Difference of p27LI between each grade was focused. Survival probabilities were estimated based on Kaplan-Meier's Product-Limit Survival Estimates method and logrank test was used to assess the association between p27LI and patient survival.Results:The p27LI ranged from 3.25% to 86.4% with a mean value 24.7%±22.0%? And in WHO Ⅱ?Ⅲ?Ⅳ,each mean was separately 38.6%±19.8%?27.8%±24.7% and 12.1%±13.3%. Analysis of variance indicated a significant difference among them (P<0.01). And the p27LI had a negative correlation with tumor grade. The Kaplan-Meier's Survival Curve shows that p27LI was statistically significantly associated with patients survival.Conclusions:There is a strong negative relationship between p27LI and histologic grade. The higher histologic grade, the lower p27LI. And low p27LI is associated with poor prognosis. In the same histologic grade there is a significant difference in patient's prognosis due to the different p27LI.
