小切口、三维固定治疗颧骨复合体骨折43例临床评价

摘    要:目的:观察小切口、三维固定治疗颧骨复合体骨折的临床效果。方法:43例颧骨复合体骨折患者经术前X线片、CT或三维CT成像确诊后,全麻下取眉弓外、同侧口腔前庭沟切口,复位后分别进行颧额缝、眶下缘、颧上颌缝三维方向的金属小钛板坚强内固定。结果:所有患者伤口均一期愈合。2例患者颧骨突度略显不足;3例患者术后张口度<3.5cm,但经过一段时间的张口训练后,张口度恢复正常;2例患者有眶下区(患侧上唇)麻木。结论:本手术法能最大限度地减少手术损伤,而且达到了三维方向坚强内固定的目的,值得临床推广。

Abstract:PURPOSE: In this study, we aim to find an effective and simple method to treat the zygomatic complex fractures. METHODS: 43 cases of zygomatic complex fractures were diagnosed with X radiographic films and CT or 3-Dimensional CT. All cases were approached with lateral eyebrow plus maxillary vestibular incision to expose and reposition the dislocated zygoma, and fixed with micro titanium plates at inferior orbital rim, at zygo-maxillary suture and zygo frontal suture, which was a 3-D fixation to offer the steady of the fractures. RESULTS: The outcome after treatment was satisfied functionally and esthetically, but little insufficient prominence of the zygoma was present in 2 cases, numbness in inferior orbital area in 2 case and fewer than 3.5 cm in open bite in 3 cases which relieved through the rehabilitation treatment. CONCLUSIONS: With this kind of treatment, the surgical injuries has been reduced, the scars were unappar ent, the fixation was stabilized in three dimensions. It is an ideal method and deserved to be recommended.
