超声诊断急性阑尾炎的应用价值及技巧

目的探讨超声诊断技巧在急性阑尾炎中的应用,以提高超声诊断准确率。方法对比分析70例急性阑尾炎患者的术前超声检查与手术病理结果,术前超声联合运用高、低频探头,并采用顺逆旋转扫查法及顺逆旋转延伸扫查法显示阑尾。结果 70例经手术及病理诊断为急性阑尾炎中,超声显示65例,诊断准确率92.9%,各型阑尾炎超声均有较高显示率（86.4%～100.0%）。盆位、盲肠后位、回肠后位阑尾炎超声显示率低于回肠前位、盲肠下位及异位阑尾。结论综合运用超声检查技巧,可提高对急性阑尾炎的检出率,具有重要的临床应用价值。

The diagnostic value and skill of ultrasound in acute appendicitis

Objective To improve the diagnostic rate of ultrasound in acute appendicitis.Methods Seventy cases of acute appendicitis were analysis with preoperative ultrasound and postoperative pathology.All cases used clockwise rotary sweeping,anti-clockwise rotary sweeping,clockwise extended sweeping and anti-clockwise extended sweeping to demonstrate appendix.Results The ultrasound can demonstrate 65 cases in this way and the accurate rate of ultrasound was 93%.The demonstration rates of all types of acute appendicitis were 86.4～100%.The ultrasound could find appendicitis that located the different positions,but the display rate of pelvic cavity,retrocecal and backward ileum appendicitis is lower than the prepositional ileum,cecum lower and ectopic appendix.Conclusion Using the skill of ultrasound can better detect the acute appendicitis and provide a good value for clinical surgery.
