Mirizzi综合征15例诊治分析

目的探讨Mirizzi综合征的病因、病理特点及临床表现,总结新的合理的诊断与治疗Mirizzi综合征的方法。方法对经手术治疗证实的15例Mirizzi综合征的临床资料作回顾性分析。结果15例均行手术治疗,手术方式分别采用胆囊切除术、胆囊部分切除术、胆囊切除加胆总管或肝总管探查术、胆道重建术或胆肠内引流术。结论Mirizzi综合征病理分型不一,术前诊断困难,需借助实验室化验检查及多种影像学检查枝术。对不同病理类型的患者应采取不同的手术方法。

Analysis of diagnosis and surgical treatment of 15 cases with Mirizzi syndrome

Objective To discuss cause of disease, pathologic feature and clinical manifestation of Mirizzi syndrome and to summarize a new, rational method of diagnosis and treatment. Methods Clinical materials of 15 surgically proved cases with Mirizzi syndrome were retrospectively analyzed. Results All cases were treated by surgical operation. The operations included partial cholecy stectomy, cholecystectomy, cholecystectomy & common bile duct or common hepatic duct exploration, T tube drainage and choledochojejunostomy. Conclusion The pathologic types of Mirizzi syndrome were diversified. It was difficult to make a definite diagnosis before operation. Varied imaging techniques and laboratory examination were helpful for diagnosing. Different operation methods should be applied according to different pathologic types of this disease.
