心力衰竭与脑钠素、C反应蛋白和一氧化氮浓度的相关性

摘    要:目的:探讨血清脑钠素(BNP)、C反应蛋白(CRP)、一氧化氮(NO)浓度等因素在不同程度心衰中的变化和相关性。方法:51例心衰患者按纽约心脏病协会(NYHA)分级标准分为(Ⅱ~Ⅳ组)3组,另设正常对照组27例。同时检测血清BNP、CRP、NO浓度,并进行多元方差分析和偏相关分析。结果:正常对照组和心衰程度各组之间的BNP(18.35±19.42)pg/ml比(62.34±67.34)pg/ml比(70.03±44.49)pg/ml比(99.87±83.08)pg/ml]、CRP(4.42±5.94)mg/ml比(19.73±36.65)mg/ml比(25.31±47.26)mg/ml比(18.20±41.53)mg/ml]、NO(143.17±78.62)μmol/L比(158.69±131.40)μmol/L比(165.29±197.46)μmol/I。比(133.93±117.06)μmol/L]水平存在着总体上的明显差异(P均〈0.05)。偏相关分析显示,血清BNP与NO呈负相关(r=-0.240,P〈0.05)。结论:血清脑钠素、C反应蛋白、一氧化氮浓度的变化,在正常和不同程度心衰患者之间,总体上存在着明显的差异及相关性。临床上检测它们,有助于诊断心衰和判断心衰程度。

Abstract:Objective: To investigate changes of serum concentrations of brain natriuretic peptide (BNP), C--reactive protein (CRP) and nitric oxide (NO) in different degrees of heart failure (HF) and their relationship with HF. Methods: A total of 51 HF patients were divided into three groups (NYHA class Ⅱ group-Ⅳ gruop) according to New York heart association (NYHA) classification standards, and another 27 cases were enrolled as normal control group. Serum concentrations of BNP, CRP and NO were measured, and multivariate analysis of variance and partial correlation analysis were performed on the results. Results: Levels of BNP ( 18.35 ± 19.42)pg/ml vs. (62.34 ± 67.34)pg/ ml vs. (70.03±44.49)pg/ml vs. (99.87±83.08) pg/ml],CRP(4.42±5.94)mg/ml vs. (19.73±36.65) mg/ml vs. (25.31±47.26)mg/ml vs. (18.20±41.53)mg/ml] and NOE(143. 17±78.62)μmol/L vs. (158. 694±131.40)μmol/L vs. (165.29±197.46)μmol/L vs. (133. 93±117.06)μmol/:] showed overall significant difference among normal control and HF groups (P〈0.05 all). Partial correlation analysis showed that serum BNP was negatively correlated with NO (r=- 0. 0240, P〈0.05). Conclusion: Changes of serum concentrations of brain natriuretie peptide, C reactive protein and nitric oxide show overall significant difference and correlation among normal people and patients with different degrees of heart failure. Detection of them in clinic is helpful to diagnose heart failure and determine the degree of heart failure.
