直肠癌切除术3种吻合方式效果比较研究

目的评价直肠癌切除术3种吻合方式的效果，为临床手术术式选择提供依据。方法回顾性分析573例直肠癌手术患者资料，比较传统手工吻合（A组）、单纯吻合器机械吻合（B组）和在吻合器机械吻合的基础上加用全层间断缝合吻合口（C组）3种吻合方式术后主要并发症吻合口漏、吻合口出血、吻合口狭窄的发生情况。结果C组患者术后3种主要并发症的发生率分别为吻合口漏0．98％、吻合口出血0．49％、吻合口狭窄1．47％，均明显低于A、B2组患者。结论在吻合器机械吻合的基础上加用全层间断缝合吻合，较单纯手工吻合及吻合器机械吻合，能显著降低术后主要并发症的发生率，是一种安全有效的术式。

Comparative study on three anastomotic procedures for resection of rectal carcinoma

Objective To evaluate the results of three anastomotic procedures for resection of rectal carcinoma so as to better deal with the condition clinically.Methods Retrospective analysis was made on 573 patients with rectal cancer to compare the occurrence of major postoperative complications after the three anastomotic procedures: traditional manual anastomosis(group A), mechanical anastomosis with stapler(group B),mechanical anastomosis combined with the whole layer interrupted suture with silk thread(group C). Results The incidence rates of major postoperative complications in group C were 0.98% for anastomotic leakage,0.49% for anastomotic bleeding and 1.47% for anastomotic stenosis,which were all evidently lower than those in groups A and B.Conclusion Supplementing the whole layer interrupted suture with silk thread to the mechanical anastomosis with stapler can lower the incidence of major postoperative complications,and so be considered as a safe and efficient procedure.
