甲状腺高功能腺瘤患者脑血流动力学变化

摘    要:目的应用TCD探讨甲状腺高功能腺瘤患者的血流动力学变化及其临床价值. 方法按常规检测32例高功能腺瘤患者颅内动脉的血流动力学指标,与30例健康正常组对照;32例患者再分为血流速度显著增高(A组)、中度增高(B组)、轻度增高(C组)3组进行比较. 结果所有患者血流速度均增高、收缩峰高尖,搏动指数正常,高代谢征候群者占70%,伴突眼者占60%.A组中症状明显者比例显著高于其它两组. 结论脑血流速度的增高程度可初步反映高功能腺瘤患者病情的严重程度,并与症状发生程度有相关性,因而TCD可用来评价高功能腺瘤患者病情的严重程度并可提高早期确诊率.

Abstract:Objective To invenstigate the hemodynamic changes and the clinical value in of transcranial cranial doppler (TCD) in detection of highfunction adenoma patients. Methods The intracranial hemodynamic indexes of thirty-two case wihe highfunction adenoma patients and thirty case healthy controls by were determined TCD.Thirty patients were divided into 3groups based upon the increased velocities : the highest v velocity group(Group A),the moderate velocity group(Group B) and the mid velocity group(Group C). Results The increase of velicities was observed in all 3 groups.The systolic velocity was high and sharp, but the pulsatility index was normal, High-metabolism were observed in 70% patients and exophthalmia were found in 60%patients. T3 and T4 in patients of group A were significantly higher than those in group B and C, while TSH of group A was significantly lower than those in group B and C; T3 and T4 in patients of group B were significantly higher than those in group C,while TSH of group B was significantly lower than those in group C. symptom were most found in group A than those in the other 2groups. Conclusion The severe extent of highfunction adenoma can be observed by the increased cerenbral blood velocities which would affect symptom.TCD is helpful for judging the severtity of highfunction adenoma and effectiveness of therapy. It may be possible to raise the early diagnostic rate also.
