尺骨鹰嘴截骨Y型钢板内固定治疗肱骨远端C型骨折

目的：探讨尺骨鹰嘴截骨Y型钢板内固定治疗肱骨远端粉碎性骨折的方法及疗效。方法：采用尺骨鹰嘴截骨Y型钢板内固定治疗肱骨远端C型骨折16例,术后随访9~16个月,平均13.4个月。采用改良Cassebaum评分系统评价术后肘关节功能。结果：优5例,良8例,可2例,差1例,优良率82%,无骨折不愈合、切口感染等并发症发生。结论：经尺骨鹰嘴截骨Y型钢板内固定治疗肱骨远端C型骨折,显露良好,复位满意,固定牢固,可早期功能锻炼,肘关节功能恢复满意。

Y-SHAPE PLATE FOR THE SURGICAL MANAGEMENT OF C-TYPE FRACTURES OF THE DISTAL HUMERUS THROUGH APPROACH OF OSTEOTOMY OF OLECRANON

Objective：To explore the surgical management and results of comminuted fractures of the distal humerus through approach of osteotomy of olecranon with Y-shape plate.Methods：sixteen patients with C-type fractures of the distal humerus were treated surgically with Y-shape plate through approach of osteotomy of olecranon,followed up from 9 to 16 months（average 13.4 months）.According to Cassebaum scoring system,the function was assessedResults：5 cases were excellent,8 cases good,2 cases fair and 1 case poor,and the rate of good outcome was up to 82%.There was no complication such as infection.Conclusion：The technique of Y-shape plate for the surgical management of C-type fractures of the distal humerns through approach of osteotomy of olecranon offers many advantages,such as suffficient exposure,satisfied reduction,stable fixation,earlier exercise and good funcion.
