天鹅型记忆接骨器与钢板内固定术后软组织界膜的生物相容性

摘    要:目的:通过局部组织大体观察、组织学病理及超微结构观察等方法,比较天鹅型记忆接骨器(swan-like memory connector, SMC)和不锈钢板内固定后对于软组织界膜的影响,初步评价SMC的生物相容性. 方法:对临床既往骨折后经SMC或不锈钢板内固定病例13例经随访发现骨折取得骨性愈合后,于内固定取出术中取与金属密切接触的局部软组织进行大体标本观察、光镜和电子显微镜观察. 结果:SMC组局部软组织结构致密规则,炎症反应较轻或无,微观结构观察见为规则致密的纤维结缔组织,成纤维细胞为主要细胞成分,细胞周围可见排列规则致密、纵横交错的胶原纤丝;钢板组软组织结构多数紊乱,局部组织发黑或发灰,微观结构观察胶原纤维增生,透明变性,结构紊乱,并有不同程度的炎性反应或异物反应(异物巨细胞反应、纤维素样坏死)存在. 结论:SMC具有良好的生物相容性,是一种理想的生物医用材料.

Abstract:AIM: To assess the biological compatibility of swan like memory connector (SMC) and stainless steel plate after internal fixation by comparing the effects of SMC and stainless steel plate on soft tissue limiting membrane. METHODS: The local soft tissues which tightly contacted metals were observed grossly, histologically and ultrastructurally from 13 cases of patients, who had achieved fracture healing and whose internal fixators had taken out. RESULTS: The local soft tissues of SMC group had regular fiber distribution and less or none inflammatory reaction. The main component cells were fibroblasts which were surrounded by collagen fila arranging in a crisscross pattern and regular array. Stainless steel plate group had disordered local soft tissues, with black or gray changes in part. In ultrastructure observation, collagen fiber was hyperplasiaed, disordered and hyaline degenerated. More inflammatory reaction and foreign body reaction (foreign body giant cell reaction and fibrinoid necrosis) were also seen. CONCLUSION: Swan like memory connector is of good biological compatibility and is an ideal biological material for medicine use.
