摘    要:目的 调查2009年我院临床分离菌对常用抗菌药物的耐药性.方法 采用纸片扩散法(K-B法)进行抗菌药物敏感试验,以WHONET5.4软件进行数据分析.结果 791株临床分离菌中,革兰阳性球菌293株,占37%,革兰阴性杆菌498株,占63%,前3位病原菌依次为铜绿假单胞菌、大肠埃希菌和金黄色葡萄球菌.MRSA在ICU、内科和外科的检出率分别为29%、18%和23%;MRSE检出率分别为32%、21%和26%,未检出对糖肽类抗生素耐药葡萄球菌属.产ESBLS大肠埃希菌和克雷伯菌属分别为48%和43%,铜绿假单胞菌耐亚胺培南的菌株占38.4%.结论 临床常见病原菌中革兰阴性杆菌约占2/3,碳青霉烯类抗生素对肠杆菌科细菌保持最高活性.细菌耐药性呈上升趋势,特别是多重耐药铜绿假单胞菌较往年有显著的增加,应引起重视.

Abstract:Objective To investigate the bacterial resistance profile of clinical isolates collected from the First Hospital or Zibo City during 2009. Methods Kirby- Bauer (KB) method was used to determine the antimierobial susceptibility. Data were analyzed by WHONET5.4 soitware. Results Of the clinical isolates, gram - positive cocci and gram - negative bacilli accounted for 37 % and 63 % respectively. The top three pathogens were P. aeroginosa, E. coli and The prevalence of MRSA in ICU, internal medicine and surgery were 29%, 18 % and 23 %, respectively. The prevalence of MRSE in ICU, internal medicine and surgery, were 32%, 21% and 26%. None of gram- positve cocci was found resistant to glycopeptides. The prevalence of ESBLS - producing E.coli and Klebsiella spp. were 48% and 43% . 38.4% of P.aemginosa strains were resistance to imipenm. Conclusions More than two thirds of these clinical isolates are gram - negative bacilli, for which catbapenenms are highly active. Bacterial resistance is still on the rise. It is important to note that the prevalence of pan - resistant P. acruginosa in 2009 is higher than that in previous years.
