认知行为疗法对精神分裂症后抑郁的疗效分析

摘    要:目的 探讨认知行为疗法对精神分裂症后抑郁的临床治疗效果.方法 对符合入组标准且完成研究的精神分裂症后抑郁患者随机分为西酞普兰合并认知行为治疗组(研究组,60例)及西酞普兰治疗组(对照组,62例).采用汉密尔顿抑郁量表(HAMD)、阳性和阴性症状量表(PANSS)评定临床疗效.结果 研究组4、6、8周末及随访1年后HAMD评分较对照组显著改善;入组前及8周末PANSS总分及各因子分两组比较无明显差异(P>0.05),但随访1年后PANSS总分及各因子分研究组明显低于对照组,差异有显著性(P<0.05或P<0.01);研究组复发率低,依从性好.结论 认知行为疗法对精神分裂症后抑郁疗效确切,有利于患者回归社会.

Evaluation of Efficacy of Cognitive Behavior Therapy in Treatment of Post-schizophrenia Depression

Abstract:Objective To evaluate the clinical efficacy of cognitive behavior therapy(CBT)on the patients with Post-schizophrenia depression.Methods 128 patients with post-schizophrenia depression were randomly divided into citalopram treatment plus CBT group(study group,n=64)and citalopram treatment group without CBT(control group,n=64).The efficacy was measured with Hamilton depression scale(HAMD)and the Positive and Negative Symptoms Scale(PANSS).Results The score of HAMD was better in study group than that in control group after treatment for 4,6,8 weeks and 1 year follow up.The total score and factor scores of PANSS in the two groups before and after treatments for 8 weeks were not significantly different(P0.05),however,the total scroe and factor scores of PANSS were lower in study group than in control group after a year,and the total score and factor scores of PANSS of both groups before and after treatments for a year were significantly different(P0.05 or P0.01).The study group had lower rates of recurrence and good compliance than control group.Conclusion CBT is effective for post-schizophrenia depression and is beneficial for the patients to return to the society.
