Castleman病1例报道及文献复习

目的：认识Castleman病的临床和病理特征。方法：报道1例以多克隆免疫球蛋白升高、低蛋白血症、蛋白尿为主要表现而无显著浅表淋巴结肿大的Castleman病的临床资料并结合文献复习，为其诊断和鉴别诊断提供一条思路。结果：患者女性，43岁，体检发现白蛋白减少、球蛋白增高。双侧颈部、腹股沟淋巴结轻度肿大，但没有进行性增大。有蛋白尿。淋巴结活检病理：血管滤泡性淋巴组织增生，浆细胞型。结论：Castleman病临床较少见，易误诊，应列为多克隆免疫球蛋白升高的鉴别诊断，确诊需进行病理和免疫组织化学检查。

Castleman disease:A case report and review of the literatures

Objective:To inquire into the clinical and pathological features of Castleman disease(CD).Method:One case of polyclonal immunoglobulinemia ,without obvious solitary mass,was reported and literatures were reviewed in order to give some suggestion for diagnosis.Result:A 43-year-old female was presented here.She had polyclonal immunoglobulinemia, hypoproteinemia and proteinuria,without obvious solitary mass. Histology revealed CD of plasma cell type.Conclusion:CD should be considered in the differential diagnosis of immunoglobulinemia.
