超声导引下经皮穿刺治疗外科术后腔内局限性积液

为了协助临床更为直接、快速处理术后并发症。对28例术后局限性感染性或化脓性积液患者行经超声导引下经皮穿刺抽吸，并以甲硝唑200～1000mL冲洗，然后注入20mL生理盐水溶解的一定剂量的广谱抗菌素。在28例患者中，胸腔感染性或化脓性积液5例，腹腔感染性或化脓性积液16例，盆腔感染性或化脓性积液7例。结果28例患者中，16例经1次介入治疗后，体温恢复正常，积液90％～100％吸收，有效率为100％；10例经2次介入治疗后，体温恢复正常，积液〉90％吸收，有效率为100％；2例经3次介入治疗后，体温恢复正常，积液〉90％吸收，有效率为100％。所有患者在治疗过程中未发生并发症和严重的不良反应。初步研究结果提示，超声导引下经皮穿刺治疗外科术后腔内局限性感染性或化脓性积液是安全有效的，且方法简便、价格低廉，应在临床推广和应用。

Interventional therapy with ultrasound-guided percutaneous puncture for postoperative intra-cavity limited fluid collection

The objective of this study was to assist surgeon to treat directly and rapidly the postoperative complication. Method: Twenty-eight patients with postoperative intra-cavity limited infected or purulent fluid collection were punctured and aspirated with ultrasound-guided. After that the cystic cavity were washed with 0.5% metronidazole injection 200-1 000 mL. Finally, an amount of broad-spectrum and 20 mL 0.9% sterile saline solution were injected into the cystic cavity. Anomy the 28 patients, 5 patients suffered from pleural infected or purulent fluid collections, 16 patients intra-abdominal infected or purulent fluid collections, 7 patients pelvic infected or purulent fluid collections. Among the 28 patients, the temperature in 16 patients recovered normal after one procedure, the temperature in 10 patients after two procedures, and the temperature in 2 patients after three procedures. No complications and serious side effects were observed during the procedure in all the patients. Conclusion: Interventional therapy with ultrasound-guided percutaneous puncture with antibiotics is a simple, safe, economic and effect therapy for postoperative intra-cavity limited infected or purulent fluid collection.
