等离子射频联合改良悬雍垂腭咽成形术治疗阻塞性睡眠呼吸暂停低通气综合征50例

摘    要:目的探讨等离子射频联合改良悬雍垂腭咽成形术治疗阻塞性睡眠呼吸暂停低通气综合征(0SAHS)的疗效。方法采用等离子射频消融术治疗下鼻甲肥大、舌根肥厚联合改良悬雍垂腭咽成形术治疗OSAHS患者50例,回顾分析临床资料及疗效。结果术后6个月显效为60%,有效为35%,无效为5%,总有效率为95%;术后12个月显效为58%,有效为31%,无效为11%,总有效率为89%;术前与术后6个月、12个月的最长呼吸暂停时间、最长低通气时间、呼吸暂停低通气指数(AHI)、最低血氧饱和度(LSaO2)差异有统计学意义(P〈0.05);术后6个月、12个月的最长呼吸暂停时间、最长低通气时间、AHI、LSaO2差异无统计学意义。结论对多平面阻塞OSAHS患者应进行联合治疗,等离子射频联合改良悬雍垂腭咽成形术可达到较好的疗效。

Abstract:Objective To investigate the clinical effect of plasma radio-frequency ablation combined with modified uvulopalatopharyngoplasty(UPPP) in treatment of obstructive sleep apnea hypopnea syndrome(OSAHS). Method The clinical data of 50 cases were retrospectively analyzed. Results At post-operative 6 months, 60% cases were significantly improved, 35% were improved, 5% were not improved, and the total response rate was 95%. At post-operative 12 months, 58% cases were significantly improved, 31% were improved, 11% were not improved, and the total response rate was 89%. The longest times of apnea and hypopnea, the apnea hypopnea index(AHI) and the lowest arterial oxygeon saturation(LSaO2 ) were statistically different between those before and after the operations, but were not significantly different between those of post-operative 6 months and 12 months. Condusion Plasma radio-frequency ablation combined with modified UPPP is effective for OSAHS.
