| 摘 要： | 目的分析改良腭咽成形术（H-UPPP）治疗阻塞性睡眠呼吸暂停低通气综合征（OSAHS）的疗效，探讨围手术期治疗方法。方法对41例患者行H-UPPP术治疗，摘除双侧扁桃体，切除腭帆间隙多余脂肪组织，完整保留悬雍垂，部分重度患者术前行持续正压通气治疗及对症治疗。结杲患者术后自觉症状均有明显改善或消失，无误呛及进食返流等症状，术后6个月经多导睡眠呼吸监测（PSG），总有效率82．9％。结论H-UPPP通过保留悬雍垂，维持咽腔基本结构，有效防止腭咽关闭不全，提高手术疗效和安全性。

| Abstract: | Objective To investigate the curative effect of peri-operative treatment and H-uvulopalatopharyngoplasty（H-UPPP） on obstructive sleep apnea hypopnea syndrome, Methods 41 patients were treated by continuous positive airway pressure for 1 week. Then tonsillectomy and H-UPPP were performed. Result PSG showed the total response rate was 82.9% on the 6th month post-operatively. All signs were improved or disappeared. Conclusion H-UPPP can effectively improve the symptoms of OSAHS and keeps the physical function of uvula and the basic construction of pharynx and prevents velopharyngeal insufficiency. |
