血CEA、AFP、CA125、CA199检测对肝癌的筛查价值

目的：探讨血癌胚抗原（CEA）、甲胎蛋白（AFP）、糖链抗原125（CA125）、糖链抗原199（CA199）检测在肝癌筛查中的作用。方法：选择笔者所在医院2010年1月-2012年10月收治的75例原发性肝癌患者、90例肝脏良性疾病患者及104例健康成人为研究对象，分别组成原发性肝癌组、肝脏良性疾病组与对照组。对所有研究对象进行血清AFP、CEA、CA125、CA199的含量检测，分析并比较几种肿瘤标志物对诊断原发性肝癌的灵敏度和特异性。结果：原发性肝癌组患者血清CEA、AFP、CA125、CA199水平显著高于对照组及肝脏良性疾病组，差异具有统计学意义（P〈0．05）。CEA、AFP、CA125、CA199联合检测与单项检测相比，灵敏度显著提高，差异具有统计学意义（P〈0．05），但特异性降低。结论：CEA、AFP、CA125、CA199联合检测可用于原发性肝癌的临床筛查，对提高肿瘤早期检出率具有重要意义。

The Screening Value of Serum CEA. AFP, CA125. CA199 for Liver Cancer

Objective： To discuss serum careinoembryonic antigen （ CEA ）, alpha-fetoprotein （ AFP ）, carbohydrate antigen 125 （ CA125 ）, carbohydrate antigen 199 （ CA199 ） detection in liver cancer screening.Method： For the study, people who were treated in our hospital from January 2010 to October 2012, 75 patie nts with primary liver cancer patients, 90 cases of benign liver disease patients and 104 healthy adults were composed of primary liver cancer group, benign liver disease group with control group.All subjects were detecting serum AFP, CEA, CA125 and CA199, sensitivity and specificity of the analysis and comparison of several tumor markers in the diagnosis of primary liver cancer.Result： Primary liver cancer serum CEA, AFP, CA125, CA199 level was significantly higher than that of the control group and benign liver disease group, the difference was statistically significant （ P〈0.05 ） .CEA, AFP, CA125, CA199 combined detection of single detection sensitivity improved significantly, and the difference was statistically significant （P〈O.05）, but the specificity decreased.Conclusion： CEA, AFP, CA125, CA199 combined detection of clinical screening for primary liver cancer, has important significance to improve the detection rate of early tumor.
