外伤性多发性颅内血肿的临床诊治分析

目的：探讨外伤性多发性颅内血肿的早期诊断和治疗。方法：对笔者所在科室2007年9月-2011年12月收治的186例外伤性多发性颅内血肿患者的临床资料进行回顾性分析总结。结果：手术148例,非手术38例,按GOS评分,死亡18例（9.7%）,植物生存状态9例（4.8%）,重残26例（14.0%）,中残23例（12.4%）,良好110例（59.1%）。死亡原因为颅内血肿量大、弥漫性脑肿胀并恶性颅高压,脑干功能衰竭等。结论：提高诊断意识、加强临床观察和动态CT扫描有助于多发性颅内血肿的早期诊断,并改善预后。

Clinical Analysis of Diagnosis and Treatment of Traumatic Multipleintracranial Hematomas

Objective： To explore early diagnosis and treatment of traumatic multiple intracranial hematomas. Method： Macle a retrospective analysis of clinical data on 186 patients with traumatic multiple intracranial hematomas who received surgical treatment from Sep. 2007 to Dec. 2011 in our department. Result： Of them, 148 cases received surgical treatment and the other 38 cases received non-operative treatment. According to Glasgow Outcome Scale （ GOS ）, 18 cases died postoperatively （ 9.7% ）, 9 cases were in persistent vegetative state （ 4.8% ）, 26 cases had severe disability （ 14.0% ）, 23 cases had moderate disability （ 12.4% ）, 110 cases had better outcome （ 59.1% ） . The causes of death included large intracranial hematoma, diffuse brain swelling, malignant elevated intracranial pressure and brainstem function failure et al. Conclusion： It is helpful for the early diagnosis and improvement on the prognosis of multiple intracranial hematoma to raise diagnosis consciousness and strengthen the clinical observation and dynamic CT scan.
