血清胆碱酯酶测定在肝硬化Child-Pugh分级中的临床价值

摘    要:目的对肝硬化患者血清胆碱酯酶活性与血清白蛋白浓度同时检测,观察二者在肝硬化Child-Pugh分级中的临床价值。方法血清胆碱酯酶采用速率比色法,血清白蛋白采用溴甲酚绿法在HITACHI-7600全自动生化分析仪上测定。结果肝硬化患者严格按照Child-Pugh分级标准分成A、B、C三级,各级肝硬化患者血清胆碱酯酶活性均下降,且各级之间有显著性差异(P<0.01),各级肝硬化患者血清胆碱酯酶活性降低与血清白蛋白浓度降低呈明显的正相关(r=0.56)。结论血清胆碱酯酶活性与血清白蛋白浓度是反映肝细胞损害的敏感指标,在对肝硬化患者的诊断、治疗、预后判定中有着重要的意义,特别是对肝硬化患者进行Child-Pugh分级划分中临床价值较大。

Abstract:Objective To determine serum cholinesterase(ChE) and albumin(Alb) in patients with cirrhosis and investigate the relationship of ChE and Alb level with Child-pugh classification.Methods With the help of HATACHI-7600 analyzer,serum cholinesterase was assayed by colorimetry and serum albumin was determined by bromocresol green method.Results The patients with cirrhosis were divided into the levels(A,B,C level) according to Child-pugh classification.Serum ChE activity declined significantly in all patients with cirrhosis compared to healthy subjects; and significant differences existed in ChE activity in all levels(P<0.01).There was positive correlation between ChE activity and Alb concentration(r=0.56).Conclusion Serum ChE activity and Alb concentration could be used as effective markers for not only judging liver cells injury but also evaluating the diagnosis and prognosis in patients with cirrhosis.It was helpful to child-pugh classification.
