熄风化痰法治疗高血压急性脑出血57例临床观察

目的:探讨中医辨证施治在高血压急性脑出血中的应用。方法:选择伴发高血压的急性脑出血患者,分为两组,治疗组57例,对照组67例。观察1个月,以神经功能缺损评分改变判定疗效。结果:治疗组54.38％的患者辨证为肝风夹痰,其次是痰热内闭和阴虚风动两型。两组总体疗效差异无显著性,而治疗组肝风夹痰型疗效优于对照组。结论:肝风夹痰型是高血压急性脑出血的主要中医证型;熄风化痰加活血通腑法可进一步提高高血压急性脑出血的疗效。

Clinical Observation on 57 Cases of Acute Encephalorrhagia with Hypertension Treated with Calming the Endopathic Wind and Resolving Phlegm Methods

Purpose: To probe into application of TCM treatment based on differentiation of symptoms and signs for acute encephalorrhagia with hypertension. Methods: The patients of acute encephalorrhagia with hypertension were divided into TCM syndrome differentiation group (50 cases) and western medicine group (67 cases), and they were treated for 6 months. The therapeutic effects were assessed by nerve functional defect rating. Results: In the TCM group, 60% of the patients were the type of liver - wind interspersed with phlegm, and next, type of phlegm - heat and excess of Fu - organs, and type of wind - stirring duo to Yin - deficiency. There was no significant difference between the two groups in total therapeutic effect, but calming the endopathic wind and resolving phlegm method for the TCM group was superior to the western medicine group in the therapeutic effect. Conclusion: The type of liver - wind interspersed with phlegm is main TCM syndrome type of acute encephalorrhagia with hypertension and calming the endopathic wind and resolving phlegm plus activating blood circulation and relaxing the bowels can further increase the therapeutic effect on acute encephalorrhagia with hypertension.
